60 Minute Intake Assessment: $195

50 Minute Session: $180

I am in-network with the following insurance providers:

Please contact your insurance carrier to verify that I am covered by your specific plan.

I recommend asking these questions to your insurance provider to help determine your benefits:

• Does my health insurance plan include mental health benefits?

• Does this coverage include services with an out-of-network provider?

• Do I have a deductible? If so, what is it and have I met it yet?

• Does my plan limit how many sessions per calendar year I can have? If so, what is the limit?

• Do I need written approval from my primary care physician in order for services to be covered?

I am an out-of-network provider for all other insurances at this time and can provide a superbill upon request. Contact your insurance company to verify out-of-network benefits if you would like to seek partial reimbursement for services.

If you are uninsured or I am not in-network with your insurance, you have the option to pay directly for your sessions instead.

Payment:

I accept all major credit cards and HSA cards as forms of payment. You will set up this payment method in your online client portal before our initial intake session. If using your insurance, I will bill your insurance first and you are responsible for any copay.

Cancellation Policy:

If you are unable to attend a session, please make sure you cancel at least 24 hours beforehand. Otherwise, you will be charged for the full rate of the session.
